What Is Lip Filler Migration?
Lip filler migration is when the dermal filler used to enhance the lips moves outside the intended area – most commonly above the top lip. This can result in a blurred lip outline or swollen appearance, sometimes referred to as “duck lips” or a “filler moustache”. It can also occur in the mucosa (‘wet area’) just inside the upper and lower lip.
How Does Migration Happen?
Migration can happen for a number of reasons, including:
- Poor injection technique
Injecting outside of natural anatomical boundaries or placing filler at the wrong depth can increase the risk of migration. Certain techniques, such as the ‘Russian lip technique’ are more commonly associated with lip filler migration. - Overfilling the lips
Lips only have a certain capacity. Exceeding this can cause filler to be pushed out into unintended areas. - Incorrect product choice
Softer fillers may spread more easily if used incorrectly, particularly in such a dynamic (i.e. constantly moving) area like the lips. Conversely, very thick products also have a higher risk of migrating over time because they take longer to break down and are exposed to muscle movement for longer periods. - Frequent top-ups without dissolving old filler
Without periodic assessment, layers of filler can build up and shift, especially with repeated injections into the same area.
While it’s not dangerous, migrated filler can cause puffiness, distortion of your natural shape, and even affect how your lips move. It also compromises the quality of your result — which is why we take it so seriously.
Why We Won’t Treat You If You Have Lip Filler Migration:
We know it can be disheartening to hear that you’re not suitable for treatment, especially when you’re looking for a top-up or hoping to fix the issue. But here’s why we won’t just add more filler:
- More filler on top of migrated product only makes the problem worse. It adds volume where it’s not needed and distorts the lips further.
- It becomes harder to achieve a clean, natural shape when filler is sitting in the wrong anatomical planes.
- We always want to preserve the integrity of your natural lip anatomy, and we can’t do that until we start from a clean slate.
That’s why, if migration is present, we’ll almost always recommend dissolving the existing filler first. This involves a quick treatment using a product called hyaluronidase, which breaks down hyaluronic acid-based fillers.
Once the area has settled (usually after 2-4 weeks), we can reassess and discuss the most appropriate way forward.
How Can You Avoid Lip Migration?
We recommend only booking your appointments with a registered medical professional with an aesthetic qualification who understands the anatomy of the face. A lot of treatments in the UK are carried out by non-medics and this can significantly increase your chances of migration and more dangerous complications.

Steps we take to help reduce the risk of migration:
- Fewer injections = fewer microchannels
Each injection creates a small pathway where filler could potentially move. We minimise this by limiting the number of passes. - Targeted filler placement
Techniques like microboluses (tiny droplets) directly into the correct compartment reduce the risk of filler “spilling” across boundaries. - Depth control
Injecting too superficially or too deeply increases the risk of lumps, bruising, and migration. Our team uses evidence-based techniques to ensure filler is placed just where it needs to be. - Product selection
We use premium hyaluronic acid-based fillers selected for their compatibility with the lips’ natural movement – helping to keep filler exactly where it’s meant to be, but importantly also allowing the use of hyaluronidase to dissolve filler if required.
This approach allows us to build volume safely and gradually – helping to prevent migration before it happens.
We also assess each patient’s lips holistically. Sometimes the best result doesn’t come from adding more filler, but from carefully shaping or treating surrounding areas for balance.
Can Filler Migration Go Away On Its Own?
In some cases, mild migration may reduce over time, especially if the filler is very soft or naturally breaks down quickly. However, once filler has migrated outside of the intended area, it rarely disappears completely on its own.
If the filler has shifted into tissue where it’s not supposed to be (like above the lip border), it often needs to be dissolved.
How Soon After Treatment Does Filler Migration Occur?
Filler migration can happen shortly after treatment – usually within weeks or months, but it may also develop gradually over time, especially with repeated treatments.
Common timelines include:
Immediate migration, due to poor injection technique or incorrect placement.
Delayed migration, after multiple top-ups or too much filler being added over time, often with no gap between sessions for the tissue to settle.
Even when lips look great at first, migration can occur months later if the filler moves or builds up in areas like the upper white lip (the skin just above your lip border).
That’s why we always stress spacing treatments out appropriately and avoiding overfilling.
Why Does Migration Most Commonly Happen in the Lips?
While migration can technically happen anywhere filler is used, it’s most commonly seen in the lips. That’s because lips are a highly mobile area – we’re constantly speaking, eating, smiling, and moving them. Combine that with thin skin, a complex structure, and the popularity of frequent top-ups, and it’s easy to understand why they’re more vulnerable to movement than areas like the cheeks or chin.
This is why lips require not just an aesthetic eye, but a medically informed approach.
Can Your Lips Go Back to Their Original Shape After Dissolving?
This is a question we hear a lot and understandably so. The good news is that, in most cases, yes, your lips can return to their natural shape once the migrated filler is dissolved. Hyaluronic acid-based filler is temporary and dissolvable, and once removed, the underlying tissue is usually left unaffected. However, if you’ve had multiple treatments over several years or experienced significant stretching, a subtle difference in texture or volume might remain. Most commonly, these changes will have naturally occurred over time through age-related transitions in our anatomy. Patients who have had lip filler for a number of years can experience a degree of ‘perception drift’ when it comes to their appearance, and so it can take some time to get used to going back to the natural baseline.
The most important thing is to allow the area time to settle before deciding on your next steps.
What to Do if You Suspect You Have Lip Filler Migration
If you think you might have migrated filler or something just doesn’t look quite right – don’t worry. You’re not alone, and we see this often.
The good news is that it can be fixed.
Book a consultation with a medical aesthetics practitioner and they’ll assess your lips, if not visible to the naked eye, an ultrasound can be used to detect if the filler has migrated. If migration is present, you will have the option to dissolve it.
