“As we age, we also see changes in bone support, soft tissue position and skin quality” explains Dr Sophie. “The lips can appear flatter and less defined, while changes in the surrounding tissues may also affect how prominent they appear.”

Understanding why your lips appear thin is an important first step in creating an effective treatment plan. For some patients, the cause is simply genetics. For others, it may be related to ageing or changes elsewhere in the face.

Can Anyone With Thin Lips Have Lip Filler?

Not necessarily.

“One of the biggest misconceptions about lip filler is that thin lips simply need more volume,” says Dr Sophie. “In reality, the lips are just one part of a much larger picture.”

During your consultation, an experienced practitioner should assess the entire lower face, not just the lips themselves. This is because the appearance of thin lips is often influenced by the structures that support them.

For example, some patients naturally have a smaller or more recessed chin. Others may have lost support around the mouth due to ageing, volume loss or skin laxity. Muscle activity around the mouth and dentition and skeletal support also affect the appearance of the lips. In certain cases, the lips can appear less prominent because the surrounding framework isn’t providing adequate support.

“When this happens, adding filler to the lips alone can sometimes create an unbalanced result,” explains Dr Sophie. “Patients may feel their lips still don’t look quite right despite having more volume, because the underlying issue hasn’t been addressed.”

In some cases, improving support elsewhere in the lower face can enhance the appearance of the lips and overall facial harmony, often with less lip filler than a patient initially expected.

This is one reason why a thorough consultation is so important. The treatment you ask for isn’t always the treatment that will give you the best result.

Why More Filler Isn’t Always Better

Many patients assume that if their lips are very thin, they will need a large amount of filler to achieve a noticeable result.

However, repeatedly adding volume isn’t always the answer. 

In fact, one of the reasons some patients develop overfilled or unnatural-looking lips is because filler is being used to solve a problem that isn’t actually caused by a lack of lip volume.

“If the lips don’t have the right support from the surrounding structures, adding more filler won’t necessarily improve the overall result,” says Dr Sophie. “In some cases, it can actually make the lips look disproportionate to the rest of the face.”

For patients with naturally thin lips, a staged approach is often recommended, building volume gradually across multiple appointments rather than attempting to achieve the desired result in a single session. This allows the lips to adapt, helps maintain a natural appearance, and gives both patient and practitioner the opportunity to assess how the lips are responding to treatment.

The most successful lip filler treatments are usually those that respect the patient’s natural anatomy and enhance the lips gradually, rather than attempting dramatic changes in a single appointment.

What If I Have a Long Upper Lip?

Another important consideration is the length of the upper lip.

Some patients have a naturally longer distance between the nose and the upper lip, which can make the lips appear thinner or less visible, particularly when smiling.

“Patients are often surprised to learn that what they’re noticing isn’t always a lack of lip volume,” says Dr Sophie. “Sometimes it’s the proportions of the area around the lips that are influencing their appearance.”

While lip filler can improve definition and add volume, it cannot shorten the upper lip itself. This means there may be limits to what filler alone can achieve.

Patients with a long upper lip sometimes find that they continue to feel dissatisfied despite increasing lip volume because the underlying concern has not changed.

In these cases, alternative treatments may be worth discussing. A consultation allows your practitioner to assess your anatomy and explain which options are likely to provide the most effective and natural-looking result.
